Genicular Artery Embolization for Knee Osteoarthritis: What Patients Should Know

Knee osteoarthritis can cause pain, stiffness, swelling, and reduced mobility. When exercise, weight management, medication, physical therapy, or injections do not provide adequate relief, some patients begin comparing newer minimally invasive options with conventional orthopedic care. One of those options is genicular artery embolization (GAE).

GAE is not right for every painful knee, does not restore cartilage, and cannot guarantee that knee replacement will be avoided. Understanding both the potential role and the limitations of the procedure is essential.

What Is Genicular Artery Embolization?

GAE is an image-guided catheter procedure intended to reduce abnormal blood flow associated with inflammation in the lining of an osteoarthritic knee. Through a small access site, an interventional radiologist guides a catheter to selected genicular artery branches and delivers microscopic particles.

The goal is to reduce hypervascularity associated with synovial inflammation and, in selected patients, improve pain and function. GAE does not replace the knee joint, correct alignment, rebuild cartilage, or treat every cause of knee pain.

What Does the Clinical Evidence Show?

The evidence for GAE is evolving and mixed. Early prospective studies and a 2022 multicenter randomized sham-controlled study reported improvement in selected patients. However, a 2024 randomized sham-controlled trial found similar pain reduction after GAE and sham treatment and did not establish a clinical effect beyond the sham response.

A 2025 systematic review of sham-controlled randomized trials concluded that GAE may provide short-term symptomatic relief in selected patients but emphasized the need for larger and more rigorous studies. Patients should be told that improvement is not guaranteed and that the long-term comparative role of GAE is still being defined.

Who May Be Evaluated for GAE?

GAE may be considered for selected adults who have imaging-confirmed knee osteoarthritis, persistent pain that affects daily activities, and an inadequate response to appropriate conservative treatment. A complete evaluation should consider:

  • The location and pattern of pain
  • Physical examination findings and arthritis severity
  • Prior physical therapy, medication, injections, or surgery
  • Kidney function, allergies, bleeding risk, and current medications
  • Vascular anatomy and the findings on prior imaging
  • Personal goals and expectations

Knee pain can also come from injury, infection, inflammatory arthritis, instability, referred pain, nerve-related pain, or another diagnosis. GAE should not be used as a substitute for establishing the cause of the symptoms.

What Happens During the Procedure?

A small catheter is introduced through an artery, commonly at the wrist or groin. Angiography helps identify genicular artery branches associated with the symptomatic area. Microscopic particles are then delivered selectively, and the catheter is removed after treatment.

Procedure time, medication, observation, and discharge instructions vary. Many cases are performed without an overnight hospital stay, but the plan depends on anatomy, medical history, access site, and the response to treatment.

Potential Benefits and Important Limitations

GAE uses a small catheter access site and does not surgically alter the knee joint. Some patients in clinical studies have reported improved pain and function. At the same time:

  • Some patients have little or no improvement.
  • GAE does not reverse osteoarthritis.
  • Pain may return, and additional treatment may be required.
  • Physical therapy, weight management, medication, injections, or orthopedic treatment may still be recommended.
  • GAE cannot guarantee that knee replacement will be delayed or avoided.

What Risks Should Patients Discuss?

Potential risks include bruising or bleeding at the catheter site, temporary skin discoloration, a pain flare, contrast reaction, kidney injury, infection, nerve or skin injury, radiation exposure, and rare non-target embolization. Individual risk depends on anatomy, medical conditions, medications, and technique. Your physician should explain warning symptoms and when to seek urgent care.

How Does GAE Fit with Other Knee Treatments?

GAE is one option within a broader treatment plan. Depending on the diagnosis and arthritis severity, care may include therapeutic exercise, weight management, activity modification, oral or topical medication, injections, bracing, or surgery. Coordination with an orthopedic, sports medicine, rheumatology, rehabilitation, or primary-care clinician may be appropriate.

  • What is the most likely cause of my knee pain?
  • Why might GAE be reasonable—or unreasonable—for me?
  • What benefits are realistic, and how will improvement be measured?
  • What are my individual risks?
  • What are the alternatives, including continued conservative care and surgery?
  • What happens if GAE does not help?
  • Is the procedure covered by my insurance plan?
